City of Bothell: Request for Statement of Qualifications 2018 Pavement Preservation – Federal Funding COB Project # 80000433.03003 The City of Bothell Public Works Department is requesting Statements of Qualifications (SOQ) from firms qualified and interested in providing professional services for the 2018 Pavement Preservation Project - Federal Funding. Project Description: The project includes replacing the wearing course of asphalt and upgrading pedestrian access facilities along 228th Street SE between 19th Avenue SE and 35th Avenue SE. The work to be performed by the Consultant consists of planning, engineering, environmental documentation, permitting, right-of-way, cost estimating, utility coordination, and construction engineering support. It is anticipated that the consultant work for this project will be authorized in phases. The selected consultant must have the capability to complete the entire design and permitting process, and provide construction engineering support services. It is anticipated that the consultant will begin work immediately on the initial design. The Consultant, at the option of the City, may also be contracted to complete additional phases, such as final design, and construction engineering services. Submittal: Submittals must include the following information: Firm name, phone number, and address; name of Principal-in-Charge and Project Manager; and subconsultant information. Submittals will be evaluated on the following criteria: 1) Capability of team leader 2) Key personnel experience with pavement preservation and ADA compliance 3) Firm experience with FHWA and LAG Manual requirements 4) Construction support team experience, including inspection. Please submit eight (8) copies of the SOQ to the Bothell Public Works Department, c/o Khin Gyi, 18415 – 101st Ave. NE, Bothell, WA 98011, no later than 5:00 p.m. (PST) on July 20, 2017. The SOQ shall be no more than 20 pages, excluding the title page. Pages containing information on both sides of the pages will be considered two pages. Consultants will be initially evaluated based on their written SOQ. Finalist candidates will be interviewed. One consultant will be selected for the project.
